2022 School Immunisation Survey year 7

Thank you for taking the time to participate in this survey.

You have been invited to take part as you are a parent, guardian, or carer of a Year 7 student and were offered vaccination as part of the Gold Coast School Immunisation Program (SIP).

About the survey

Gold Coast Public Health Unit is currently reviewing the delivery of the School Immunisation Program. This survey is being conducted to understand your views, opinions and experiences of how vaccination services are delivered in a school setting.

How will the information be used?

The information collected from this survey will be used to inform program planning and service provision. This will help us to be a leader in compassionate, sustainable, highly reliable healthcare.

About your participation

Your participation in this survey is voluntary.

Whether or not you choose to participate, your access to services will not be impacted in any way. This survey has been designed to be anonymous, meaning your answers cannot be linked back to you and you will not be identified in any report.
This survey will take approimately 5 minutes to complete the first part. There is a second component of the survey that will take approximately 5 minutes as well. If both sections are being completed, this survey will take approximately 10 minutes in total.
